What we collect
What you type into our forms: your name, email address and, where you choose to give them, phone number, ZIP code or city, organization and program details, and your answers to the questions on the form. We also record which page the form was sent from and any campaign tags in the link you followed.
We don't ask for children's names, medical information or immunization records on this website, and we ask you not to include them.
How we use it
- To contact you about early access, the founding provider group or a partnership you asked about.
- To decide where to launch, using ZIP codes and cities in aggregate.
What we don't do
- We don't sell or rent your personal information.
- We don't share it with other providers or partners without your permission.
- This site doesn't use advertising cookies.
Service providers
Form submissions are stored by the service that hosts our forms and may be managed in our business systems. These providers process the information only on our behalf.
Keeping and deleting your information
We keep submissions until launch and for as long as you want to hear from us. To see, correct or delete what you sent us, or to stop hearing from us, email hello@enrollpass.com.
Changes
We'll update this notice as EnrollPass launches, and change the date at the top when we do.
